“Just Culture in Human Factors”

Kent Stauffer, FlexJet

In this complex world of ours, how do we ensure that we come to work fit for duty and promulgate Just Culture in our organization when we are only human?  We will explore the inner workings of the mind to help discover what contributes to our errors and offer some helpful tips on how to overcome our limitations to maintain fitness for duty and Just Culture.

11:00 a.m. –12:00 p.m.

“Runway Suprises – Excursions and Incursions”

Roger Cox, Retired NTSB Senior Air Safety Investigator

Pilots are usually surprised when they have an excursion or incursion, even when the risk profile is high. Better policies and training can prepare them for that risk. Using well-researched examples from 2017 to 2023, I discuss the risk profiles of mostly corporate jet aircraft involved in runway excursion and incursion accidents. I Identify key takeaways from each case and suggest company policies that could have reduced or eliminated the risk in these cases.

“Overcoming the Hype: Ushering AAM into Operational Reality”

Bryan Willows, Bristow Group

This session will introduce Bristow’s AAM efforts, why our history of operational execellence and Target Zero safety culture is critical to the emerging AAM sector, and how we work with our OEM partners to develop aircraft that are safe, efficient, and reliable so that we can prove to regulators, communities and the flying public that AAM has the potential to revolutionize the way we travel regionally.

2:00 p.m. – 3:00 p.m.

“Building a Culture of Excellence Through Proactive Compliance”

Jermaine Cadogan, Convergent Perfomance

Presentation designed to identify the five primary reasons for noncompliance and identify the seven most costly errors that plague aviation maintenance technicians. Using your experience and collective wisdom, we will be able to determine the conditions that cause noncompliance to occur in your organization or team and how you can collectively prevent and eliminate the Never Events. Lastly, you will discuss potential solutions for noncompliance in your organization and identify things you can do individually to raise the bar for yourselves, your organizations, and the aviation industry.
3:00 p.m. – 3:30 p.m. Refreshment Break Fuel up with afternoon coffee, tea, water and a light snack.
3:30 p.m. – 4:30 p.m.

“YOUR OPERATION AND SYSTEM THEORETICAL ANALYSIS”

Florida Institute of Technology

Whether or not it is formally recognized, safety analysis is dependent on robust hazard analysis. Using Boeing as a case study, this presentation will demonstrate how system theoretical analysis could be used to identify organizational problems, including those that involve system design as well as policies and procedures and cultural issues. Prospectively, the presentation will then show how system theoretical analysis can also be used to proactively identify problems before an accident happens.

Important COVID-19 Information

COVID-19 is an infectious disease caused by the novel coronavirus. Some variants of the virus can cause severe symptoms including acute respiratory issues, severe damage to lungs and other organs, and even death. According to the U.S. Centers for Disease Control and Prevention (CDC), COVID-19 is thought to spread mainly from person-to-person, and individuals over age 65 and people of any age who have certain underlying medical conditions are at higher risk for severe illness from COVID-19. Understanding of the symptoms, risks, and spread of the disease may change over time, especially if new variants arise.

Although ACSF has selected a venue where reasonable safety precautions will be taken to protect the health of participants, ACSF cannot guarantee that participants will not be exposed to COVID-19 during attendance at the event.

All participants acknowledge (1) that participation in this ACSF Safety Symposium and the group activities is completely voluntary, (2) there are inherent risks of exposure to COVID-19 while traveling or participating in group events such as this, and (3) that, for themselves (and for their heirs and assigns) they assume all risks associated with COVID-19 in connection with their attendance and participation, including risks to their own health and the health of others with whom they may come in contact in connection with the ACSF Safety Symposium.
